Hospice nurse combines caregiving and creativity to raise funds

A dedicated nurse at St Giles Hospice has crafted a special 'Elephant Bear' to support the hospice's March of the Elephants trail.

Jane Mogford, who has been nursing for over 30 years with more than two decades at St Giles, has been creating sought-after artist collectable bears as a hobby for the past 25 years.

Her handcrafted bears typically sell for around £150 each and have found homes across the globe.

Jane's collectable bears are unique, handcrafted artworks using high-quality traditional materials, unsuitable for children under 14 due to their delicate nature.

Her latest creation, "Effie the Elephant", stands at 25cm tall. Crafted from lavender viscose fur with ultra-suede footpads, Effie features glass eyes, needle sculpting, and hand-shaded details. She's weighted with steel shot, 5-way jointed, and adorned with a purple chiffon ribbon and velvet flower.

For Jane, bear-making is more than just a hobby—it's a form of mindfulness and a creative outlet that helps her cope with the emotional demands of her role in palliative care.

Jane said: "There's just something special about artist bears. A lot of time, love, and a little bit of the artist's soul goes into each creation. It's one of my stress relief techniques. When I started working on the oncology ward, dealing with younger patients and all the psychological distress, it took an emotional toll. Bear-making allows me to switch my mind off from work and focus on creating something positive."

Inspired by the March of the Elephants arts trail, Jane has donated Effie to raise additional funds for the hospice, directly benefiting the patients she cares for daily.

She said: "I thought, let me make an elephant bear, and that will be my contribution to this project. It's amazing to think that something I've created could raise money to support our patients. That's exactly why I've donated it."

Staff Education an Anchor Amid Widespread Hospice Audits

Hospices need a three-pronged approach to prepare for, respond to and appeal audits as more providers come under a wider regulatory microscope.

A wide swath of auditing activity has ramped up in the hospice space as regulators seek to improve program integrity and quality oversight. More than half of hospice providers reported having multiple types of audits within a six-month span in a survey earlier this year.

This trend means that hospices have increasingly been preparing for or addressing several audits at different phases of the process, according to Jason Bring, co-chair of post-acute and long-term care at the law firm Arnall Golden Gregory LLP (AGG).

“Audits are now a normal part of business for hospices and home health agencies,” Bring said during the podcast. “Having an audit with denials is really routine at this point in the industry. The auditors are coming in and looking for any reason to deny and then rubber-stamping the denials that were done at the lower levels. You expect and think that they’re going to take a fresh and fair look at things.”

Supplemental Medical Review Contractor (SMRC) and Targeted Probe and Educate (TPE) audits are among the most common types of audits that hospices undergo simultaneously alongside others.

Responding to and appealing these types of audits has become a more time- and resource-intensive process with potential effects on hospices’ operations and margins, according to Bring. Hospices need to have a detailed approach when examining an audit notice in order to understand how to respond, particularly in SMRCs, he added.

Having ongoing, internal clinical documentation and billing review processes is key to both avoiding and responding to the common red flags on auditors’ radars, which often hinge on around patient eligibility and services related to their terminal condition, Bring indicated.

“SMRCs used to be a little bit kinder and gentler, but now are more serious,” Bring said. “Audits have a lot more danger potential because they’re taking away your money, potentially hundreds of thousands of dollars. You need to protect yourselves as much as possible from the audit risk, and doing these routine checkups is super important.”

Auditing readiness and responsiveness are significant parts of not just compliance, but also sustainability, said Megan Turby, vice president of quality and compliance at Gulfside Healthcare Services.

Two large beacons that remain a constant in compliance are staffing education and documentation, but hospices’ approaches to each of these may need revamping, Turby stated.

“The audits start and they do not stop,” Turby said during a recent podcast . “One of the biggest things I’ve taken away from all of it is to think of auditing prep as survey readiness. You want to have good education in place. You want to see [staff] paint the picture. You’re supporting everything on the backend in the background to be paid properly and defend what you’ve done. You want to be ready all the time.”

Gulfside Healthcare Services, a nonprofit, provides hospice, home health and palliative care from 11 locations across two counties on Florida’s west coast near the Gulf of Mexico. The hospice provider recently underwent an audit appeals process.

Staff need a firm understanding around the potential red flags on regulators’ radars and how to avoid common compliance errors in their roles, Turby stated. Compliance training should be focused on staff’s overall responsibilities alongside the larger bottom line of quality, she said.

Data is paramount to both a hospice’s audit response and preparedness, according to Dr. Lisa Barker, chief medical officer at Gulfside Healthcare Services. Documentation discrepancies can even among interdisciplinary teams as they report on a patient’s condition, Barker stated.

“One thing we saw that judges brought out is sometimes there was a discrepancy between what the nurse documented and what the nurse practitioner documents in a face-to-face [visit],” Barker said. “The big reason for that is because at the time when those notes were constructed the patients can have a change in condition. [It’s] really going back to your team and trying to understand what the discrepancies are. Make sure you explain that in your [physician] narrative, that’s very important.”

A hospice nurse is raising awareness about the certain less than pleasant realities which typically occur when someone passes away.

A hospice nurse has opened up about what can happen to people's bodies 'immediately' after they pass away.

The nurse, who goes by 'Hospice Nurse Julie' on YouTube , uses her platform to talk about all things 'hospice care, death and dying' in the aim of educating, helping prepare people and overall 'decreasing the fear of death'.

But unfortunately, death sometimes comes with some pretty grim realities on the body.

Dying is an uncomfortable topic and one many often shy away from talking about, however, it's important to be aware of certain realities that come with someone passing away so you don't get any unexpected, nasty surprises during what can already be a very emotional and challenging period to navigate.

And so licensed hospice nurse Julie has taken to YouTube to raise some awareness about what happens to the body 'after it dies'.

Well, 'immediately' after someone passes away, Julie explains the body 'relaxes' and this can lead to some pretty 'messy thing[s]' occurring.

Julie explains as soon as someone passes, their body 'relax[ing]' can cause that person to 'urinate, have bowel movements, sometimes have fluid come up their nose or out of their eyes or nose, ears'.

"All of the things in your body that are holding fluids, relax," she continued. "That's why death can be messy sometimes after someone dies."

The nurse clarifies this isn't 'always' the case 'sometimes the body relaxes so much it releases all of its fluid'.

But why is it so important we all know this pretty gruesome and quite alarming fact?

Well, Julie said: "I like to talk about it so people aren't surprised if that happens. Very normal and to be expected sometimes."

And the leaking of bodily fluids isn't the only thing which happens to a body when the person passes away either, Julie noting 'body temperature drops' too.

However, how fast this occurs after the person passes can differ, some people becoming cold 'immediately' and others taking a bit longer.

And depending on how long they've been lying there, you may notice the back of their legs and body develops a 'darker tone of skin' because 'all their blood is pulling down'.

Something else to be aware of is how quickly rigor mortis kicks in, causing the person to become 'stiff' - the time frame, once again, varying from person to person, sometimes stiffness beginning in minutes and other people taking longer.
