U of A team working to improve health care access in Alberta and beyond

University of Alberta master of science student Pulkit Kumar discusses the wide-ranging ways he and his colleagues are working to revamp health care access in the province and across the world.

This interview has been edited for clarity and length.

Michael Higgins: As the Alberta government overhauls the provincial health-care system, a University of Alberta medical student is on a mission of his own to bring about change within the health community, and not only here in our province, but globally also.

You have a very lengthy list of pursuits but let's start quickly on some of what you're doing right here in our province. What are you aiming to achieve where rural surgical care is involved?

Pulkit Kumar: This project is launched by the Office of Global Surgery here at the University of Alberta under the guidance of Dr. Abdullah Saleh, who's the director of the office, and The idea is basically to have enhanced surgical skills (ESS) and obstetrical surgical skills (OSS). So we're going to train family doctors with ESS and OSS and send them to rural Alberta, which will in the long run help the residents of rural Alberta to get basic surgery done where they are living, instead of driving two to three hours just to get to Edmonton or Calgary to get some very minor surgical procedures done. We are also hoping to get the surgical wait times reduced by quite a bit if we get this project up and running soon.

MH: And I understand too, AHS is kicking in some dollars toward this?

PK: Not only AHS but the province as well. So we have about $10 million for this project, a yearly grant of $10 million that is going to be put into this project, and we're hoping to have this project as a long-term solution, not just as something short term. We at the Office of Global Surgery always focus on long-term sustainable goals, so that's what we are hoping to achieve with this project.

MH: Now there has been considerable focus and concern of late over the sustainability of cancer care services in Alberta. To what degree does your research reach into that realm?

PK: So I can talk about the U of A Cancer Society, which I founded in 2020. The idea of that student organization was to push forward more research amongst undergraduate students and the other aspect was to educate people more so about cancer. I've been talking about this to numerous journalists, that When I went to rural Alberta I saw some very, very shocking treatment of cancer patients with their family members. So one of the stories that still is in my mind was that when I visited a family who had a cancer patient, they sort of cornered the patient to the room and said, ‘We think cancer is airborne,’ which was really, really surprising for me. We were like, ‘We don't know where you got to know that information from but we just want to tell you cancer is not airborne and we would like you to read more scientific papers, more scientific discussions that are happening around cancer, and we will help you read those papers by removing all the scientific jargon.’ And so in the long run, my idea is to build critical reading in these communities so that people are well informed as to what is going on in terms of medical research. When it comes to sustainability in cancer research, I'm obviously part of academia, I focus on research in academia, meaning that academia is a little slow when it comes to the industry, but we are treating any disease holistically. We are not just focusing on one aspect of the disease, we are treating everything holistically, which takes a bit of time, and I'm certainly OK with that because if it is taking time, that just means we are making sure that we are covering all the bases of the disease that we are focusing on.

MH: We're talking about Alberta Health elements, but you also do stuff globally. What role do you ultimately see for yourself in the health care dynamic?

PK: My job is to just positively contribute to the environment, to the system. Many people when they start, they are very enthusiastic and they often say, ‘We want to change the system, we are here to change everything.’ I'm like, there's a reason the system is in place. But my job is to look at the gaps, look at the voids that are in the system, and then sort of address the concerns, address those voids, fill out the gaps. So globally, even with my work with South Asia, what I'm doing is looking at those gaps in the medical system of those respective countries, and then I'm looking at how we can improve the system and how we can fill those gaps. For instance, in Nepal, there's like five to six communities that are living in rural parts of Nepal where they have no health care, no local clinics. So we sort of collaborated with local (non-governmental organizations) there and we built a mobile health care unit, a mobile surgical unit, and sent them to those communities so that we can help them, at least in the short term. And in the long term we're going to plan and build at least two clinics per community so that they don't have to travel 100, 150 kilometers just to get some basic surgery done.

The Post Anesthetic Care Unit provides comprehensive care in a critical and highly specialized area to post-surgical patients as they recover from general and/or regional anesthesia. The successful incumbent will gain valuable nursing experience working alongside skilled staff, in a fast paced and dynamic environment, supporting AHS’s commitment to providing safe, high quality, family centered care.

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. Strong interpersonal & problem solving skills. Proven reliability/dependability. Adaptable to a fast-paced, rapidly changing environment with high acuity patients. Ability to deal with unpredictable and stressful environment. Ability to maintain a physically demanding job. 2 years of Phase 1 Recovery and/or critical care experience. Advanced knowledge of regional anesthesia, neuroaxial anesthesia, multimodal analgesia and chronic pain. Must be able to work staggered shifts, possibly extended shifts.

At least 4-6 months current PACU experience within a tertiary setting. Previous orientation/preceptorship for new staff and students. ACLS. Experience with patient care quality improvement/quality assurance. Canadian Nurses Association in Perianesthesia Nursing, Critical Care, and/or Medical/Surgical Certification. National Association of Perianesthesia Nurses of Canada membership.

Alberta’s Health Research Ethics Boards

Alberta has three Research Ethics Boards (REBs) designated under the Health Information Act to review and approve health research. Each one is housed within a sponsoring organization.

REBs are independent in their decision making and are accountable to the highest level in the organizational body that established them for the process of research ethics review. These organizations are accountable for the research conducted under their jurisdiction.

Health Research Ethics Board of Alberta (Alberta Innovates)

The Health Research Ethics Board of Alberta (HREBA) is the provincial board that brings together the former-CPSA board, the community board, and cancer board into one with three committees with the following names:

HREBA – Clinical Trials Committee (CTC) 

  • Formerly known as the Research Ethics Review Committee (RERC), the CTC protects the public by enabling scientific and ethical review of research proposals submitted by registered Alberta physicians, who are conducting research involving human subjects

HREBA – Community Health Committee (CHC)

  • Formerly known as the Community Research Ethics Board of Alberta (CREBA), the CHC is a multi-disciplinary committee with members from both rural and urban Alberta that reviews health research being conducted in communities across the province.

The  Katz Group Centre for Pharmacy and Health Research  is one of two distinct entities which together make up the University of  Alberta’s Health Research Innovation Facility   (HRIF) : a world-class research and teaching facility. Current occupants are conducting research in viral immunology, biochemistry, cardiovascular and gastroenterology, to name only a few. The facility also has a 220-seat lecture theatre, teaching and learning centre, imaging facility, glass wash facility and Vivarium. Designed to be flexible and modular, the facility maximizes efficient use of space and supports adaptation to future needs.

This building provides the main entrance to the complex and becomes the critical link between the Medical Sciences Building and the Heritage Medical Research Centre. The lecture theatre, teaching and learning centre and a food outlet are located within the Atrium, which encourages interaction amongst researchers and provides important links to existing circulation systems. The included classrooms accommodate a wide-range of multi-media, audio-visual, data communication requirements, and instructional-related laboratory spaces for students and collaborative meeting areas.

The buildings are connected to the existing Heritage Medical Research Centre and the Medical Sciences Building and have become part of a complex of interrelated facilities, which together form an impressive health sciences research complex. The buildings are home to the Faculty of Medicine & Dentistry; the Faculty of Pharmacy & Pharmaceutical Sciences; the Faculty of Agricultural, Life and Environmental Sciences. Assessments, redevelopment, and upgrades were undertaken in adjacent facilities including the Medical Sciences Building and Heritage Medical Research Centre.

At the Collaborative Health Research Institute of Southern Alberta, we aim to produce high impact health research with the potential to enrich health across the lifespan for people living in Southern Alberta and beyond. Through community engagement, a focus on patient needs, and targeting critical phases of life, our vision is to create a knowledge base for supporting healthy life trajectories among individuals, families, and communities from diverse backgrounds. CHRISA will serve as a hub for establishing transdisciplinary collaboration between researchers, service providers, and patients To advance the vision and goals of CHRISA, we welcome partners in support of our initiatives:

  • To create a platform for transdisciplinary, high quality research through effective communication and networking;
  • To enable collaboration with a variety of community partners through engagement with diverse groups in urban and rural contexts;

The AWHF supports women’s health research undertaken by the Women and Children’s Health Research Institute  (WCHRI).

Affiliated with Alberta Health Services, the University of Alberta and the Stollery Children's Hospital Foundation, WCHRI facilitates research activities, builds capacity, and catalyzes research innovation and excellence, and supports the development of evidence-informed practices, policies, and protocols in women’s healthcare across Alberta.

More than 160 researchers in women’s health are affiliated through WCHRI.   This work is directly supported by the AWHF.

What areas of Alberta does the Alberta Women's Health Foundation fund research?

The Alberta Women's Health Foundation (AWHF) funds research conducted by the Women and Children's Health Research Institute (WCHRI) located within the University of Alberta. The Institute supports over 160 researchers from across the province of Alberta working together in all areas of women's health.

Partnership in Action

The AWHF has partnered with the University of Calgary in the creation of the Lois Hole Hospital Cross-Provincial Chair in Women's Mental Health Research. Led by Dr. Dawn Kingston, this research is taking place in women's health facilities in Calgary, Red Deer, and Edmonton and seeks to better understand the impacts of anxiety and depression in pregnant women and their correlation to post-partum depression. The partnership has resulted in the creation of The Hope Digital Mental Health Platform, which offers screening, referral, and e-therapy services to perinatal women. Over 3,500 women across Alberta have utilized the platform thus far.

How does funding by the AWHF supporting WCHRI research affect healthcare delivery from across the province and beyond?

Provincial Impact

Funds raised by the AWHF support the work of WCHRI, which is not limited to the geographic boundaries of the Edmonton area. Research conducted by WCHRI researchers may include strategic research alliances with other universities or other provincial, national, and international research bodies and authorities.

National Impact

In addition to research happening here in Alberta, the AWHF and WCHRI make up the Albertan arm of the Women’s Health Collective Canada (WHCC). The WHCC was founded by the country’s leading women’s health foundations, Alberta’s own AWHF, the BC Women’s Health Foundation , and Ontario’s Women’s College Hospital Foundation to amplify our impact — and break the barriers holding us back from providing women with equitable healthcare. Through intentional collaboration, we’re bringing resources and fundraising together to address the wider gaps in women’s health we can’t tackle on our own.

The WHCC is united in:

  • Funding medical research that addresses the unique needs of women.
  • Raising awareness on the gender health gap, dispelling misconceptions about women’s health, and highlighting how healthy women create a healthier Canada.
  • Dispelling misconceptions about women’s health to help everyone know the facts.
  • Broadening the scope and reach of world-class women’s health programs to reach every woman, everywhere.
  • Creating higher standards of care for every woman.

What processes does the AWHF use to ensure donated dollars are used to create a province-wide impact?

There are many competing priorities and new emerging gaps in women’s health and women’s health research. The AWHF actively seeks out the most urgent issues to be as effective as possible as quickly as possible. Recently the AWHF launched the first in its series of province-wide surveys and subsequent report, Finding the Fractures: The Pandemic, Women's Health Disparities, and the Path to Equity to better understand the highest priority issues citizens are now facing and inform future research priorities and funding designations.

Following a survey conducted in 2022, the AWHF launched its second thought leadership report entitled  Surveying the Silence: Exploring the Impact of Taboos in Women's Health . The organization plans to continue running similar surveys and reports in the future.

How does care provided at Alberta's only dedicated women's hospital affect care for women outside of Edmonton?

OBIX Perinatal Interface System

In May of 2022, the Lois Hole Hospital for Women (LHHW), located in Edmonton, AB, installed the OBIX Perinatal Interface System, a state-of-the-art technology designed to integrate a mother's medical documentation and history with fetal surveillance in real-time. Connected to the LHHW and satellite hospitals, this technology allows care providers from across the province to communicate instantaneously with experts at LHHW, saving precious time for mother and baby. By providing remote access to leading care providers at the LHHW, patients throughout Alberta can have easy access to excellent perinatal care—without the need to leave their home location.

Allard Hereditary Breast and Ovarian Cancer Clinic The Allard Hereditary Breast and Ovarian Cancer (HBOC) Clinic at the LHHW is the only clinic of its kind in Alberta. The clinic provides risk assessment and surveillance for women throughout the province of Alberta who are at a high risk for developing breast and ovarian cancer. Women with genetic mutations to the BRCA1 and BRCA2 genes have an increased risk of developing breast and/or ovarian cancers prior to menopause.

Indigenous Cultural Partnership In collaboration with Indigenous community organizations from across Alberta, the NWT, northern BC, and northern SK, the LHHW is piloting the Indigenous Cultural Partnership program. This program will advocate for and seek to establish equal representation and prioritization of Indigenous health and wellness knowledge systems/practices in sexual and reproductive health alongside the Euro-Canadian approach that is already in place. LHHW will be the first tertiary care hospital in Canada to implement this program. The AWHF, through WCHRI’s research and program evaluation, will ensure that this LHHW initiative will influence sexual and reproductive health practices in other hospitals throughout Alberta and, in fact, Canada, to close gaps and inequities in healthcare for Indigenous patients.

Health Research Ethics Board of Alberta (HREBA)

HREBA safeguards the rights and welfare of individuals who volunteer to participate in research by ensuring ethical principles have been considered by researchers and applied prior to and throughout the conduct of the research.

The Health Research Ethics Board of Alberta (HREBA) is hosted by Alberta Innovates and consists of three committees – the Cancer Committee, the Clinical Trials Committee and the Community Health Committee. These committees provide ethical reviews and ongoing ethical oversight of cancer and community-based research involving humans, their information and biological samples.

While focused on the protection of research participants, HREBA wants to ensure that research maximizes benefit while minimizing harms.

Core principles

HREBA applies core principles of the  Tri-Council Policy Statement – Ethical Conduct for Research Involving Humans  and other applicable regulations (i.e. HIA, Health Canada, FDA, ICH GCP) which includes:

  • Respect for Persons
  • Concern for Welfare

Committee makeup

HREBA’s committees are composed of a cross-disciplinary group of individuals including doctors, members of the scientific community, non-scientific members with specific expertise, including ethicists, lawyers, privacy experts and community members from across the province.

These committees meet monthly to review all cancer research and research conducted by private practice physicians/organizations which are not under the oversight or have an existing agreement with the University of Alberta or the University of Calgary.

Non-Research Projects

Although non-research projects don’t require the approval of an REB, they still involved people and their information and therefore similar ethical principles should be considered.

A pRoject Ethics Community Consensus Initiative (ARECCI) provides support to project leads and/or organization in assessing the ethical risks of projects that don’t require research ethics approval. Visit the  ARECCI page  for more information.
